At Phelps Health Homecare, we believe that the final stages of life are just as important as other stages. Our team provides medical and nursing care, and emotional, spiritual and bereavement support for patients and their families. Our nurses are on call all day, every day to assist patients and their families.
Hospice responds to a patient’s needs for comfort, empowerment and self-directed care, and supports the patient's loved ones both during the illness and after death. The goal is to maintain the highest quality of life while retaining patient and family dignity to the greatest extent possible.
Phelps Health Homecare’s Hospice Team
Hospice care is delivered by an extensive team of professionals and volunteers. The patient, family and doctor are vital parts of the team, participating in decision-making and care. Members of the Hospice Team include the following:
- Bereavement counselor (provides support for up to 13 months)
- Homemaker/bath aide
- Hospice medical director
- Medications, medical supplies and equipment
- Social services
- Specialized therapies (occupational, physical and speech therapies)
The Phelps Health Foundation’s Hospice Fund provides financial support to qualified hospice patients and their families by assisting with prescription medications, personal bills, food supplements and other needs.
