The Transformation of Rural Community Health (ToRCH) program, sponsored by Missouri’s HealthNet Division (Medicaid), supports rural communities in tackling the nonmedical factors that shape health outcomes. Known as social determinants of health (SDOH), these factors include safe housing, transportation, access to healthy foods and more. ToRCH establishes community-based hubs that coordinate efforts between healthcare providers, community-based organizations (CBOs) and social service agencies. Learn more here.

Phelps Health leads the ToRCH program in Phelps County and oversees the program model, distributes funds and manages data reporting. With help from the ToRCH program and strong community partnerships, we’re working to remove barriers and build a healthier future for the people we serve.

How Can ToRCH Help?

The ToRCH program can help with the following:

  • Transportation: Non-medical, health-related
  • Mental health support
  • Substance use support
  • Environmental modifications to living area (homes and apartments only)
  • Nutrition support or medically tailored meals
  • Chronic condition management (such as diabetes, asthma or high blood pressure)
  • Physical disability support for living areas (such as ramps, equipment or home modifications)

     

Do I Qualify for ToRCH?

To qualify for the ToRCH Program, you must:

Have MO HealthNet (Medicaid coverage) and
Live in Phelps County, Missouri
