What is ToRCH?

The Transformation of Rural Community Health (ToRCH) program, sponsored by Missouri’s HealthNet Division (Medicaid), supports rural communities in tackling the nonmedical factors that shape health outcomes. Known as social determinants of health (SDOH), these factors include safe housing, transportation, access to healthy foods and more. ToRCH establishes community-based hubs that coordinate efforts between healthcare providers, community-based organizations (CBOs) and social service agencies.
