Homecare services are available to individuals of all ages with new or chronic conditions who meet the following guidelines:
- You are homebound.
- You are under the care of a physician.
- You require services on an irregular basis.
- Your physician orders the type of services to be provided.
- Your requested services are approved by your insurance company.

Medicare, Medicaid and most managed care and private insurance plans pay for home care services. Our staff members are available to assist you and your family in finding financial resources as needed.
Phelps Health Home Health and Hospice services can be used through a variety of settings, whether you are at home or are being discharged from a hospital or a skilled nursing facility.
