At Phelps Health Home Health and Hospice, we believe the final stages of life are just as important as other stages. Our team provides medical and nursing care (our nurses are on call all day, every day), and emotional, spiritual and bereavement support for you and your family. 

Hospice is a belief of care that responds to a patient’s needs for comfort, empowerment and the fulfillment of personal healthcare wishes. Hospice supports a patient's loved ones both during the illness and after death. The goal is to maintain the highest quality of life while retaining patient dignity to the greatest extent possible.

Phelps Health Hospice Team

Hospice care is delivered by a complete team of professionals and volunteers. The patient, family and doctor are important parts of the team, participating in decision-making and care. Members of the Hospice Team include the following:

  • Bereavement counselor (provides support for up to 13 months)
  • Chaplain
  • Homemaker/bath aide
  • Hospice medical director
  • Nursing
  • Nutritionist
  • Social services
  • Specialized therapists (occupational, physical and speech therapies)
  • Volunteers
